Colorectal cancer is a scourge. It is the second most common cancer killer in the US (third in the world), right after the infamous lung cancer:

And while CRC rates had fallen for decades with smoking rates, they are now rising for two reasons. The first reason is that old age has traditionally been a big risk factor, and the country/world are graying:

The other is that younger people are getting the disease far more frequently. This has been happening for a long time and the trend is growing:

And now the cold truth about CRC cancer – the worst cases are “cold tumors”.

The immune system is so potent that harnessing its power is often better than trying to work beside it. Thus, the checkpoint inhibitor drugs have been an enormous breakthrough. Cancer cells can send “checkpoint” signals to T cells telling them not to recognize antigens on cancer cells. Checkpoint inhibitors block this communication, allowing the T cells to do their job. 

BUT, we just described “hot” tumors – tumors that sound the immune system’s alarms. But what cold tumors? Cold tumors tend to be surrounded by cells that suppress the immune response:

And cancer’s icy heart has frozen over the hopes of many CRC cold tumor patients: 

So that is one of the reasons CRC is so deadly – the traditional checkpoint inhibitors don’t work (well) because 95% of metastatic CRC is cold (microsatellite stable).
